Registration & Forms

Requirements

All players must be current members of US Lacrosse.

Proof of current membership is required and should be submitted at the time of registration.

All players must submit a current and completed physical examination form, signed by a physician, stating that the player has been examined and is fit to participate in lacrosse.  Current is defined as being completed since the completion of the previous lacrosse season which is normally in May of each year.

All registration forms, U.S. Lacrosse proof, and medical examination form must be submitted PRIOR TO the player participating in team practice or games.

Team Fees

Fall Fees

  • $—/Player
  • Player fee covers practice field rentals, practice pennies for team, tournament fees, and miscellaneous travel expenses for coaches.
  • Travel expenses for parents and players are not included.
  • Please make check payable to “West Knox Warriors Lacrosse Club.”

Spring Fees

  • $300/Player
  • Player fee covers practice field rentals, game field expenses, official fees, equipment fees, clothing, and miscellaneous travel expenses for coaches.
  • Includes Winter “Mayhem in the Mountain” tournament.
  • Travel expenses for parents and players are not included.
  • Please make check payable to “West Knox Warriors Lacrosse Club.”
